About Infectious Disease Institute (IDI):
The Infectious Disease Institute, established within Makerere University, is a Ugandan not-for-profit organization dedicated to strengthening health systems in Africa. We focus on infectious diseases through research and capacity development to improve health outcomes across the continent.

Duty Station: High Volume Facilities, various districts including Kampala, Uganda

Key Responsibilities:

  • Collaborate with OVC community structures, District-Based Teams, RHITES, and PSWs during health outreaches to ensure effective tracking and linkage of newly tested positive individuals into the continuum of care.
  • Facilitate monthly referral and linkage meetings among community and health facility staff to review service uptake performance.
  • Provide technical assistance to PSWs and project structures on referral and linkage services within the community.
  • Liaise with PSWs to remind PLHIV (People Living with HIV) beneficiaries of their clinic appointments.
  • Guide new referrals through the healthcare system and provide health education to newly diagnosed HIV-positive patients.
  • Participate in scheduling appointments, responding to provider questions, and maintaining ongoing patient tracking.
  • Engage in HIV patient screening activities, data entry, and tracking of HIV testing services.
  • Follow up with re-engaged clients to ensure retention in care and provide additional support as needed.
  • Strengthen documentation of referrals and linkages, maintaining up-to-date files for all referrals.
  • Work with facility staff to keep beneficiary files updated and generate lists of beneficiaries due for Viral Load Tests.
  • Compile lists of beneficiaries not suppressing viral load for adherence support and counseling at the community level.
  • Complete and share monthly cascade reports with the supervisor and collaborate with the M&E team to ensure accurate data collection.
  • Attend health facility meetings and provide feedback from field observations and community structures.
  • Provide feedback on referrals made and services received, and keep an up-to-date referral file and register.
  • Follow up with community structures on positive clients missing clinic appointments and support their linkages back to care.
  • Link positive beneficiaries to psychosocial support platforms at both facility and community levels.
  • Act as the contact person between health facilities and community structures for referrals and linkages.
  • Participate in monthly case conference meetings with child protection stakeholders and share updates on referral status.

Academic Qualification:

  • Certificate (Minimum of Ordinary Level Certificate or equivalent).
  • Training in Social Work, Community Development, Public Health, Sociology, Project Planning, Community Health, or Child Protection is an advantage.

Person Specification:

  • Minimum of 3 years’ experience in community work within government institutions, health facilities, NGOs, or USAID-funded programs/projects.
  • Skills in service delivery programs, referrals, linkages, and OVC activities.
  • Excellent written and verbal communication and interpersonal skills.
  • Ability and willingness to travel within supported districts.
  • Fluency in applicable local languages and English.
  • Ability to work in a multi-disciplinary team with frequent travel.
  • High standard of personal and professional integrity.
  • Computer literacy is essential.
