1. Obtain application forms (PSF3Revised 2008) from:
   • Office of the Public Service Commission, Kampala
   • District Service Commissions country-wide
   • Chief Administrative Officer’s Offices
   • Town Clerks offices
   • www.psc.co.ug
2. Submit filled forms (3 copies) with:
   • 3 recent coloured passport-size photographs
   • CERTIFIED copies of all relevant certificates, transcripts
   • Copy of National ID
3. Send applications to: The Secretary, District Service Commission, P.O. Box 293, Masaka.
4. Serving officers should route applications through their Heads of Departments.
5. Ensure to indicate reliable Postal Addresses, Telephone contacts, and email addresses on the application form (PSF3).
